Anyone know what would cause and index file to get unintentionally dropped?

If you did a DIR of the directory or file to check it out and missed the "I" in the command, thus entering DR dirname\filemask you would be instructing BR to DROP the file rather than display it.

There was a thread about this about two months ago.

if it we're on the F: drive and you typed it's full path and hit enter it would be Freed, and then if you had the ol' ,use,kps=x,kln=x in your open statement, it would remake it blank.

(previous reply seems to have gotten lost so I'll post it again)

Ctrl+A during index processing will (I believe) leave an empty (or corrupt) index file behind.
